Contructive Improvments for "MY" gameplay

Let me start by saying I generally play "Pantheon" top and "Pantheon" jungle. Now I do play others but he is my main. I am not saying that I am perfect and have used the link posted to help me improve and have recently jumped to silver 2.

My biggest problem that I am sure of is knowing when to team play, I generally hit 250ish cs by the end of the ranked. Generally positive in kills, and help other lanes when I can. I guess my biggest question is how do I generally help other lanes and is there a general time when "team play>lane". Thanks for any advice everyone has :).
