Item balance is the worst it has ever been.
They got rid of Deathfire Grasp, which was balanced with a long cooldown, then added a whole list of extremely short cooldown, far worse, damage spikers. Namely

This shit is ruining the game. Fights are not even fights anymore. You just disappear instantly from absurd burst damage that can't even be countered properly with tank items.
The damage in the game is way too high. Fights should last way longer. And the reason for this problem is the awful items that add huge damage spikes.
