Why in every fight I take ture damage
Every single time I get into a fight I take true damage, either it's from items, minions, tower shots etc. Like why? Did they make in the last patch that everything does true damage now or what?

Can you give an actual example of this? were you on someone who built D.D?
Not all true damage is "true" damage. Often, it's dealt when they don't want damage to get mitigated twice. For example, Zoe's sleep. Waking someone up with damage deals extra damage, up to the damage dealt by the initial sleep, but that damage was already mitigated by MR. The true damage portion just makes sure your MR doesn't reduce the bonus damage multiple times.
Ignite, Smite, Cheap Shot, and Elder buff are ways for all champions to access true damage.
