i play garen mid went 16/2 and got an A+

It is because you are matched to similar other Garen players' scores. Since Garen is EZ and Broken an S is really hard, whereas if you go 5/4/10 with bard top you get an s+

My best advice is to try them in different game modes. I got an s/s- on Garen in Hexakill TT. (pretty good game mode for Juggernauts)

not sure about a 16/2 game, but I found a 15/2 game in your match history. a little over a hundred CS in a 30 minute game isn't that impressive. You might want to work on your CS.

Just make sure you're getting enough CS, especially in the mid game. As Garen this shouldn't be too tough since once mid game hits you can just run ahead of your minions and spin on the wave to kill all of the minions in one combo.

I don't really focus on CS (and in fact often sacrifice it for map pressure) but I still manage to get S-'s pretty frequently with like 12-4 or 11-3 etc. S's are a little tougher though, and S+ is pretty much once in a blue moon for me.
