Start machete and Rejuv beads (your Q gives you enough sustain to not need pots, but you need to pay extra attention for invades, as you will get significantly low). Once you get enough gold, back and get Tiamat. From there get Cinderhulk into Titanic Hydra, and then build tank items from there.

I prefer to take my Q, then E, and then W. Put points into W until you're level 5. Once you put a point into your ult, max Q > E > W. What this does is give you much better chasing and attack speed against lower targets, with very little damage loss in exchange.

Your masteries should be 18/0/12, taking Fervor of Battle and the extra healing boost in resolve.. For runes I prefer AD reds, scaling MR blues, and Armor yellows, with Attack Speed Quints.
