Lag Spikes Ruining Game Expereince

So my lag problem started (I think at least) with the server move. When I heard that they were going to be moving the server I was generally excited (as I live in Chicago), but I found that when Riot moved the server, it had created many problems. Many people were having terrible lag and connection issues, but as we know they had solved most of them within 3 weeks after the move. I did have the problem where I was experiencing insane lag spikes and excessively long disconnections, but after about a month the problems subsided for the time being. My ping went from around a 70 to a very nice 12 average, but there were (and still are) some problems. Every so often, maybe once or twice a game, I would get a brief lag spike of around 300-400 ping but it didn't bother me as much because it usually only lasted about 10 seconds. Not until recently I have been having a very detrimental issue to my connection to the game. When I start a game it will usually load normally and I would spawn, but about 5 or so minutes into the game (sometimes even earlier) I start to receive terrible lag spikes. These lag spikes are so severe that it usually ends with me dyeing do to inability to react or myself having to slowly walk back to base to avoid execution. Although these spikes do very greatly in time and severity, it makes it near impossible to play, especially if it is a ranked queue. Right now the spikes usually go from my normal 12 ping to anything in the range of 300-40,000 ping and anywhere from only lasting a few second to some lasting around 3 minutes; it may also end in me disconnecting from the game forcing me to have to exit out of the now frozen game and reconnect. I receive these spikes almost every single game and during these games it happens almost regularly (about every 5 minutes a pike will occur). I have thought about my internet and its connection to the game but I do not think it is my internet speeds fault, but I could also be wrong. I have AT&T as my provider and have pretty regular connection to the web at any time. My downloads and updates for the game are normal and have normal speed also. It may also be helpful to note that this almost exact problem was happening to me about 2 months ago but subsided for the time being. I am not sure what I can do to fix this issue, but any feedback at all would be extremely helpful so that I can finally play the game normally again.
