question about runaan's hurricane

it has come to my attention that runaan's hurricane bolts do not always proc, whether the target is in range or not

is there a specific condition that has to be met?

I noticed the problem with minions because at times it is a last hitting wonder and should be a damage spectacle against champions, but when only the primary attack happens, it cuts 50% of the item's power

I have this problem too! Did something change in recent patches? I always love to rush runaan's as Teemo for extreme waveclear and hilarious teamfights.
