Okay, this makes me believe you do not know how Vlad works.
As Vlad, the large, overwhelming majority of the time you do not end up in range of somebody unless they want to be in range of you past laning. That hasn't changed with this rework. If he reliably had a target for empowered Q, people wouldn't near-universally be saying he has problems.
You're generally not going to be close enough to someone that they would not be able to just walk away from your E before you can cast it. The delay on the nova means if they aren't within ~70% of its max radius, they can get out of range before the nova hits them.
One of Vlad's largest issues has always been lack of impact because he can't get to where he wants to be efficiently. His ult is really the only part of his kit that's ever been truly reliable because of this, as his team can make use of the amp, and he might be able to clean up after it pops if they don't kite back too far.
Vlad has no way to either force people to let him in range or encourage people to get in range of him. THAT's the counterplay that's out of his hands. He is one of the only champions in the game that cannot enable him/her self via kit. It was an issue prior to the rework, and it was multiplied after.
Also, a lot of the counterplay atm stems from how clunky his kit is, which isn't good design. It should be what the enemy does that outplays him. Not what Vlad is prevented from doing because of mechanical aspects of his kit that aren't even intended anymore.
