I'm not much of a computer dude when it comes to hardware; best I can recommend is doing some research into either a new computer in general or just see about how new parts would work.

Most possible is that the video card is dying and you need new one as a possibility of the chips of the video card are becoming desoldered since it was not able to keep itself cool enough

It is also possibility of Faulty RAM stick I personally had similar problem. So lets say you got 2 rams sticks then use only 1 ram stick

There is probably dust in the video card's heasink which prevents the card from cooling. You have to get the dust out of the Heatsink that alongside with it's fan keeps the card cool and generally keep your pc dust-free

Its really important to keep your PC cool PCs should have 1 fan that blows air into the pc and 1 fan that takes the hot air out of the pc.
