Technically modifying the game isn't allowed and a custom skin is modifying the game. I am however completely unaware of any time that anyone has been banned for using a custom skin, even highly popular streamers use custom skins now and again, and to my knowledge they've never seen so much as a warning.

Yes and No. Riot doesn't like you to mod the game at all. But as long as it doesn't affect the other players, they usually don't mind. But, it is still a bannable offense.

I think Riot's policy is that

  1. it can't affect the other players or what they see
  2. it can't give you any kind of advantage in the game, i.e. something that replaces a spell effect or a champion model with a perfect representation of their hitbox to make it easier to identify on the screen and dodge. if it passes both those tests, it's still in a gray area and they could ban you if they felt like it, but the probably wouldn't as they have no reason to.

Riot does not ban for normal custom skins.

No one can seem them but you. In fact, it's entirely shown within your client and the server is completely obvious to it. So I doubt they could even tell who is using them in any way they currently have available. So I doubt you'd need to worry about getting in trouble for using them. But keep in mind that it may mess up your game with each patch in a way that can be gotten around, but it'll be extra effort on your part to keep the custom skins working each patch.
