[CRASH] All day and night

I have not played league between the dates of December 3rd and December 15th. When I played on December 16th the patch 4.21 had been released. I download a patch and hop into the game. Everything went smoothly, I joined a lobby, selected a champ, entered the game, played for about 3 minutes . Then received the message "Attempting to Reconnect", after waiting 2 minutes I disconnect and attempt to reconnect only to see this message Screenshot(25).png. I close the client altogether then try to reconnect once more only to find this http://i.imgur.com/8BPo7m8.jpg . When i do reconnect the game is unplayable. My champ won't move and everything is frozen. I have taken steps to solve this problem. This has happened for 6 games now. And often times I have to leave a game that I reconnected to because it is frozen only to receive a LEAVER penalty Here's a list of what I have tried. 1: Repair League of Legends 2: Uninstall and re install League of Legends 3: Disable firewall 4: Switched from Ethernet connection to Wi-fi 5: Initiated a system restore ( Deleted all apps and downloads, and set all computer settings to default) 6: Initiated a system wipe (Deleted everything) 7: Prayed

PLEASE. Help me. I don't know what else to do
