What do you have to do for a S- game

I think it's because you only placed 5 wards in that game. You gotta ward!

Probably CS.

The system is skewed towards CS quite a bit. You can farm CS all the way to high As or even S, but if your CS is even only slightly low you're instantly out of consideration of S rank regardless how you do everywhere else (only exception is support, they typically make up by having extremely high CC and vision score). Gotta farm.

CS counts a lot towards it

You have to participate (and perform well in) all aspects to the game.... Wards count. You barely warded at all. No pinks. Destroyed nothing. You literally didn't use one of your 'Active Items' all game which is the Yellow Trinket btw. You shouldn't be confused why you didn't get the score reserved for top 5% of performances in a game when you sat on 2 stacks of your trinket most of the game.
