Can surrender votes not go through?

So I was just in a ranked game (ignore my bad build and etc.. I am low elo no need to point it out) anyway as you can see the vote says 4-1 yet the surrender vote does not go through. I am just wondering if this is possible or if something went wrong within this game. Any helpful responses are appreciated! Thanks :))

Only if it's not past the 20 minutes mark. You would need a unanimous vote then. If it's past the 20 minutes mark then you would only need 4.
