It should, I can't imagine how useful that actually would be. Gnar is too tank and too hybrid to benefit that greatly with that much AS.
Yes, it works on all on hit effects, even on such that count hits like Master Yis passive, Braum, Twisted Fate, Gnar, Xin Zhao, Vi, !!!VAYNE!!! (seriously, want to see broken damage, get a Vayne with that Item)
