See, the thing is, If the enemy does not have life steal and you have high health, they will always kill themselves before you. There are plenty of situations that it works great getting it first item or second item, if second item then the first had better be sunfire cape.

imo you shouldnt buy thornmail at all, unless youre malphite or rammus. there is really no point in buying a 2250 gold item when its basically negated by a last whisper and a vamp scepter.
If you build thornmail at all it has to be before the enemy adc gets lifesteal. Many adc cannot build that before IE and possibly two AS/crit items and LS. So depending on what your intention is, and who actually kills you thornmail as second or third item is just fine. Remember only in edge cases though not as core build unless you are rammus against twitch.
