Feels good to open up that handy dandy death recap and see that the enemy jungler put his cursor over my champion and pressed a button, then right clicked and then I proceeded to lose nearly 1/3 of my health bar in true damage

He didn't smite you and it dealt that much damage to you. He was auto attacking you for the full duration, refreshing the true damage ticks.
It sounds far more likely like the death recap is just fucked up.
It's the ability for it to re-up as the jungler is attacking you.
