Question regarding patch to worlds

So I've been playing LoL for a while and I just don't understand how teams get to worlds. Can anyone give me an explanation? From what I understand (which could be completely wrong),

There are two splits - Spring and Summer. The top... X teams make it to the group phase which of course just started, and the top... two teams from each group make it to world knockout stage? Then it's simply best of 3 until worlds which is best of 5?
