Lost a Lux S Token
A while back, I played Lux and earned an S+ that game. In the post-game lobby, it displayed the notification that I had recieved one but never showed up in my Hextech.

Are you sure it was the display for a token or just the message that you can now get them? Once you reach level 5 you get a message that says you can gain tokens.
