Riven's Passive Bugged?

I was playing around with Riven in a Custom AI Bot game. Is her passive bugged? If I hit Q 3 x's in a row, I immediately lose all my stacks of Runic Blade on the 3rd Q. Also her E and W were not granting her a stack of Runic Blade. It's been a while since I played her, but I thought ALL of her abilities were supposed to grant her a stack of Runic Blade.

Edit: After testing again and paying closer attention, it appears to be a display issue. The stacks disappear over the skill bar, but still display with all of the other buffs above her portrait. The E & W stacks will display over the portrait as well, but they do not display above the skill bar.
