[GAMEPLAY] Weird re-make bug!! Please investigate this

I was playing with two of my friends, one of which was playing Talon. He was having trouble with the game since he couldn't see anything including himself, champions, minions, or any health bars. As the game progressed, we got the option to re-make, but nobody was AFK. However, my friend Talon got the punishment and was considered the one who was AFK even though he had gotten to level 2 and was in lane when it asked to re-make? It makes no sense to me and should be investigated so it doesn't happen to players who are suspended or punished for innocent bugs like these.
