Fiora Multiple Passive Vital Bug

Fiora's passive vitals have sometimes been bugging showing multiple vitals on 1 target when not activating her ultimate, it baited me into going in for a kill thinking i could hit multiple vitals and deal a lot of damage while healing back enough health to win a 1v1 fight, when i auto attacked then they wouldn't pop and it made me lose the fight, it makes it really frustrating because it's hard to tell which vital will actually work and which ones are fake, just costed me and my team an entire game because of my enemy laner snowballing off that 1 lost 1v1.

I saw it around 3 times in that game.
