Quest bug?

this is the quest name and how to get complete it.

Merciless shadow : get 10 assists with a chaos icon equipped 0/10 i just finish my first win with chaos icon to get the one blade one purpose: quest with 8/6/19

some how i didn't finish the Merciless shadow or even get 1 assists for the quest report any idea how to fix it? or am i only do 1 quest at a time?

.... i think its just 1 quest at a time even tho i got both quest at the same time. just had to play another game to get it.
