Every time I play Xerath, which isn't very often, my Q stops working in the middle of the match. Seeing this is the first ability I max, it just screws me and I become next to useless. The only pattern to it I find is that it's always with Xerath, and every time I use him. Can Riot please address this issue?
Have you tried putting it on quick cast + quick-casting it + taking it off quickcast?
It happens to me a lot of times with Vi's Q (still waiting for that bug fix Riot). This is one of the workarounds that works every time for me.

I have used Xerath a lot and I have never had this glitch. I always play with q on quick cast though. Switching to quick cast should fix it, and if you want you can switch back. I highly suggest using quick cast though, you still get the indicator while you are charging the ability so you know the range and hitbox unlike other spells on quick cast.
