In champ select but still in queue?

I was just queued for Flex with two of my friends when the queue popped so we accepted, but we were put back in queue so we thought someone just declined. Then we started hearing the ticking that we would normally hear in champ select and two other people were in our chat and couldn't cancel queue, so one of us closed out the client and we were removed from queue, plus he ended up losing LP and was given a dodge timer. Nothing else stood out about it but it was clearly a bug.
