Game crashing every other game

As often as once every other game, it will crash and I will be unable to reload into the game. It usually happens around 5-15 minutes into the game, I will suddenly hit 500 ping and then be stuck at reconnecting indefinitely. When I close and reopen the client, I load to 100% and then get an error message simply saying "You have disconnected" or I won't even get to the loading screen, stuck with what's shown in the picture.
