Sion's Q, W, and Ult not working

Last game I was playing as SIon and from when I put my first point into W, things went downhill. The whole game I couldn't stack health with W, but I could use the shield. By mid or late game I could no longer activate Q or R; they just wouldn't respond.

I had the same problem multiple times on sion... only with Q & Ult though, W was fine for me... but this is seriously frustrating and match-breaking...
