Vi Q bug
Have had this happen on multiple occasions now. I was Vi and in the process of charging up my Q when I was killed, and after respawn my Q wouldn't work. I had to close down the client and reconnect before my Q would be usable again.

I've had the same problem twice now, in two ARAMs in the last two days. It's awful, I was doing well, got interrupted charging my Q, and the skill was disabled the rest of the game. Made me just about useless... I imagine it would be even worse in a normal game, as your ganks would be a joke.
