[Gameplay?] Not Receiving Token when getting Mastery Level 5 and S Rating in the same game

To start off, I have had level 4 mastery on the champion (Syndra) until today. I had also received an S rating in the game that topped it off and got me to mastery level 5. I was alerted of 3 things. First that I had increased the champion mastery from four to five, then that I had received a token for getting an -S or higher (I got an S), and then that I qualified for my series (unimportant). I was told that I received a token; however, I did not. I was playing in Summoner's Rift so I know I should have received a token. I'll check if maybe the loot hasn't been updated, but I did restart the client and the token has still not shown up. Wondering if this has happened to anyone else, also.
