Kicked for not Banning or Locking in a Champion

I have been kicked multiple times (Only twice so far) for not banning or locking in a champion. The problem is that it kicked me before the timer was up:

First time within the first two seconds of banning Second time around ten seconds into banning

This has only happened on this account, may be coincidence, and not any of my other accounts (I have around ten smurfs). Please note that they happened on separate occasions, not consecutively. It did leave me with a rather large wait after I 'dodged' a game I already waited fifteen minutes for.
