Serious In-game bug report

When playing with I was unable to use my Q after being hit in middle cast by ult. I had to play the entire rest of the match without my Q, I tried to ALT+F4 and re-open it, no luck. I hope someone take a look on this, perhaps it could happen even with another Dashing moves.
