lux quick cast ult.
it was very weird. team got in a team fight and i couldnt ult. im using quick cast all and it worked previously...after i reset i could ult normally.

It's not really a bug, but its an HUD bug.
You know how you sometimes ult before you die, and your ult fails to fire because you died while casting it?
Your ult is on cooldown, but the interface says its not. That's the bug.

nope, it doesn't go on cooldown unless you cast it! much like with aurelion, morgana etc..
