I recently played a game as 
. In the loading screen I noticed that his skin name appeared strange. After the game I went to the champion tab in the summoner profile and his name appeared as "**Kogmaw". When I entered his champion profile his skins did not show their their names above them. To test the bug again i started a custom game and I received the same results.
I also noticed in an AI game that instead of Reading as "Kog'Maw Bot" the name on the scoreboard said "#Champion_name - Kog'@Maw" Something to that effect.
I just played a game with a kog,
in the end game stats his named said " **kogmaw"
Yes, same problem on MAC client.
This bug has been mentioned in the super huge bug fix thread stickied at the top of the boards. Don't worry guys, Rito knows about their goof, and hopefully they're working to fix it soon.

It's a known bug.
The real issue here is that someone would actually play pre-rework sion in PvP, amirite?
