In all seriousness, the death recap is known to be extremely buggy, and yes, this is definitely a bug. It would be nice if it got fixed eventually. I got damaged by my brother's passive :(

Death recap is as reliable as a cotton swab condom. Probably 5 times less reliable actually. If I had to guess, it always will be since to make it 'better' is to rewrite the entire thing as they've admitted. I do not see them ever taking that time. No one leaves League of Legends over Death recap, so there is 0 income for fixing it.

I frequently see Death Recap showing people's basic attacks as having dealt true damage (when they don't have anything for on-hit true damage) or just random damage seemingly from nowhere (only listed source is the champion that used it, not the ability itself, and no icon).

As it's been stated, the death recap is buggy and unreliable. I'm pretty sure everyone has a story involving something ridiculous popping up in it. Just yesterday I was damaged by Veigar E. Not only is E his stun, but there was no Veigar in the game, so yeah.
