Trapped in the Champion Select screen

So I'm trying to play a match and get to champ select and some issues come up. First: I can't select my summoners. They are stuck on whatever I have them set to last. Second, I can pick a champ but sometimes not the skin. It'll stay stuck on one skin or another. Third, some of my teammates claimed they couldn't even pick a champ at all. After the countdown timer runs down, it's stuck in the champ select screen until the screen goes black. This has happened twice within the timespan of 10 minutes. Is this an issue with the new update or something else?
