Katarina W doesn't activate

Happened once. No idea how to repeat.

W created a dagger. The dagger landed in the ground and I was able to walk through it, I was actually standing still for most of the time- waiting for it to proc. It completely disappeared without activating, and I never even left it's radius.

The udyr smacking my face noticed it do nothing too.
