I sent in a ticket already, but just now, my client froze last second in champ select and upon logging in, I lost 23 LP and got a ~25 min queue block. I hadn't dodged any games recently so this is out of the ordinary to say the least.

This looks like the penality for leaving the game/not connecting to the game. I think your team did remake.
