What's going on

I click to play in ranked. I wait. Finally que shows up and I accept. Timer runs the full time with someone not accepting. I return to the que. I wait for about 90 seconds and am immediately pulled into the game that I clicked accept for before but was told someone didn't accept only to find out that I was the one who didn't pick a champ because apparently they went through the fastest ban phase probably on record XP Now can someone explain why I was told that someone didn't accept and then was pulled into that game 90 seconds later only to be kicked for not picking a champ? Did i glitch something somewhere? I'm going to try and see if I can repeat the process.
