[Client] If I hit "Play Again" it does NOT mean that I want to go into a Custom Game

Hasn't happened to me past 2 patches but it was very frequent up until then. I'm surprised it hasn't been patched yet ;P

Happens about twice a day for me. Usually only with a few friends playing in a group. Normal 5's blind pick is often the game type.

This has happened to me a number of times. It seems random as far as when it happens. Its not 100%.
