Major Quinn bug - vault gets you stuck in walls, repeatedly, happens extremely often over a wall

Just watch the video, this issue has been around for many patches and I have already reported it but now I have a video to prove it.

Only way of escaping is using E. While stuck you cant do anything except smite and use abilities.
