Tear with 0 stacks :(

I've had this happen more than a few times now, but not always. Essentially, if I have my tear fully stacked and I die, it'll come up with saying I have no stacks on it. This corrects itself immediately once I spawn again but its still disconcerting in the heat of battle... and was a little confusing in terms of if I should/shouldn't upgrade the tear. So far I've managed to get this to happen with Nidalee, Zillean, Swain, Nami & Ryze. Anybody else notice this too?
