Sometimes it doesn't turn on.

Sometimes it's on but the swirly thing that lets you know it's on is off. (and with chaotic teamfight and ult running, looking for poisontrail is sub-optimal)

Also, sometimes his Fling doesn't work, and you just run up and auto them.
