Queue dodge pentalty for accepting matches?

Riot please fix this. I was pressing the play button, and 2 seconds after starting the queue, the game was literally closing my session on its own and returning me back to the main screen. I pressed play again and after trying to find game, the same thing happened.

I restarted my client, started another queue. I accepted the game and after a while, for some reason, the game not only assumed that I declined instead, but also gave me a penalty for "declining too many games". I literally did not decline any game whatsoever.
