Penalized for getting an error when selecting my champion

Thanks to some 'error has occured when selecting your champion' error and a laggy client I was unable to lock in my select in a ranked lobby got the insta 5 min Q penalty, who knows maybe after waiting in Q for 5 mins on another lobby and accepting the match offer I'll just get the 'you did not accept match' bug even though I was one of the first to click accept. (which has happened 6 times to me so far, with no penalty issued yet but surely one is building up)
