This can be achieved if you set up a 5 man Queue, not sure how else it would happen

ive had this happen with a 5 man group. 2 got mid, 2 got top.

If you're in a 5 man it allows everyone to pick any position they want in the pre lobby setup and gives it to them, so if you had 3 people pick mid, one top, and one jungle, it would give each of them their selected position and you'd have 3 midlaners.
