Diamond looking to join a 5s team

I've been playing league since season 1. I've been Diamond for a few seasons but I hate solo queue so i usually get to 4 or 5 and quit climbing. I'd much rather play ranked 5s and experience true teamwork. I am comfortable playing any of the three roles mentioned in the title. I can play almost any champion on the game confidently. I will list a top picks pool below. I'm looking for a team with plat+ players only please. I can also be a sub for a team. I'm available most week days during the afternoon and evening, eastern time.

Jungle: Vi, Elise, Lee, Shyv, Gragas Mid: Anivia, Orianna, Cho'Gath, Xerath, Zed Support: Thresh, Morgana, Janna, Lulu, Leona

Agent if you have not found a team, or looking for others to join up with for Season 6, I know many Plat/Diamonds who I can refer you too. I am not a high elo player but I am helping my high elo friends build/join teams for S6.
