Looking for more for ranked 5's

I'm looking to start a ranked 5's team and we currently need three more players. I want to get ranked rewards for ranked 5's but first and foremost I want people to have fun and look at this as a learning experience. I would rather have everyone be Silver or higher with some upper bronze (1 and maybe 2) being acceptable as well. We currently need a jungle, mid laner, and top laner. Anyone who is a "toxic" person to either his teammates or the enemy team will be dropped. Format for application is as follows:
