How do I start a first blood 1v1 and is this possible without ending and getting a leaver buster before 20?

Oh what is this "start a first blood" you speak of
No no no you get first blood and get another kill so the enemy is stuck at level 1 when you reach 6
You could make a custom game, but that's about it.
