I can confirm that we have confirmed three continents.

As far as I remember the continents go like this:

Valoran - Demacia, Noxus, Freljord, Mount Targon, Piltover, Zaun

Ionia - All of its provinces

Southern Continent - Shurima, Icathia

Various Islands (Not a continent but whatever) - Serpent Isles and Shadow Isles
Said southern continent has yet to be named in any outwardly facing lore.
