What champions have a passive that turns one into another?

I think does, with his AP to Health passive.

Galio
50% MR-->AP

Vladimir
1 Ap--> 1.4 HP and 40 HP --> 1 AP

Kassadin
20% Magic damage -->Attack Speed

Dr.Mundo
0.3% Max HP --> Health Regen

Malphite
10% max HP --> Passive shield

I think that's all of them.......unless you count passives on abilities, there's a few more there, albeit a bit more of a gray area.

Edit: Forgot Garen
0.4% max health --> Health/sec out of combat

Thanks.
