Report a player out of game
I meant to report a summoner for a mildly serious offense but left the after game menu, I do remember his summoner name though.

Depending on what the offense was, and whether you have a screenshot of it, it may or may not be possible to submit a ticket to Player Support. If it is something that is generally handled in the post-game lobby, then they will often tell you that it is not something that they will handle, and will close the ticket. However, for the more serious offenses (think really serious) they may look into it.
if you have logs of the event op a support ticket and fill out the info to the best of your ability try to find the exact game in question(time stamp) and the players name
For future reference, take screenshots or video evidence and submit it through a private support ticket.
